Transitions
Programme

The Transitions Programme supports young people aged 16-23 in understanding and addressing their behaviours. Our practitioners provide five semi-structured sessions, exploring factors such as childhood experiences, identity, social connections, sex and relationships, and difficulties transitioning to adult life. We also discuss their online presence and its impact. With the young person’s permission, we offer a sixth session with their caregiver, providing insight into the work completed and guidance on how to offer continued support.

After an initial consultation, clients are usually able to start the Transitions Programme within two weeks. ​

When they have completed the programme clients receive a completion letter and access to ongoing support until the outcome of their investigation is known.
